RETURNED MATERIALS AUTHORIZATION (RMA)

Please clearly fill out all parts of this form and submit the RMA using the button at the end of this form, before sending the equipment for calibration and/or repair. An RMA code will be generated automatically and displayed in the response email. DMA will email you the estimated cost of the task to be carried out and shipping instructions.

WARNING:

Returns without RMA code will be refused and returned at the shipper's expense.

Your data will be automatically processed, therefore you will be responsible for the correctness of the information provided which will be used for all future activities (required tasks, shipping, billing...).

With the ISO17025 calibration it is possible to request the State of Conformity by sending an email to service [at] d-marchiori [dot] it in order to agree on the technical specifications, feasibility and costs.

** The request for urgency authorises DMA to perform any necessary tasks without waiting for approval of the cost estimate, and may require an extra charge. In any case, you will receive the final offer after completion of all necessary tasks.

*** A costs estimate can only be evaluated after careful inspection of the equipment by our technicians. By selecting 'No' , you authorise DMA to repair/adjust/calibrate the equipment, as required on this form, without waiting for your explicit approval. In any case, you will receive the final offer after completion of all necessary tasks.

  • The RMA is valid for 30 days from submitting this application, unless otherwise agreed.
  • Shipping charges and all risk of loss and damage associated with the shipping are the responsibility of the customer. All shipments are ex our factory, unless otherwise agreed.
